Tear trough filler lifts the hollow groove under the eye that creates the appearance of fatigue and shadow. It addresses structurally caused dark circles — not pigmentation. This is one of the highest-risk filler areas and requires a consultation before scheduling. At Forever Young Spa, it is performed with cannula technique and hyaluronidase is on-hand for reversal.

Tyndall Effect
Superficial HA placement in the thin periorbital skin causes a visible bluish discoloration. Prevented by deep, precise placement. Correctable with hyaluronidase.
Vascular Occlusion Risk
The periorbital area has a complex vascular network. Inadvertent intravascular injection is the most serious risk. Cannula technique and slow deposition reduce this risk significantly.

Conservative Product Amounts
Overfilling is the most common error in tear trough treatment. We use 0.25–0.3ml per side as the standard starting point, with 2-week follow-up assessment before adding more.
Tear trough filler treats the hollow groove that forms between the lower eyelid and the upper cheek — the area commonly associated with looking tired, dark circles, and under-eye hollowing. The tear trough is primarily a volume and structural issue: as the periorbital fat descends and the bony orbit becomes more prominent with age, the groove deepens and shadows the under-eye area. Filler placed precisely in the tear trough lifts the hollow and reduces shadowing — it does not treat pigmentation or skin texture, which are separate concerns.
Under-eye filler is safe when performed by an experienced injector — and it is one of the highest-risk filler treatments when performed by someone without sufficient training and anatomical knowledge. The periorbital area has a dense vascular network and proximity to the eye. Risks include: Tyndall effect (bluish discoloration from superficial placement of HA filler); vascular occlusion if the filler is inadvertently placed in a vessel; and lumpiness. Tear trough filler requires very small amounts — typically 0.5 to 1 syringe total for both eyes. Overfilling the tear trough is the most common mistake in under-eye treatment: too much filler migrates, creates puffiness, and produces the Tyndall blue discoloration. Conservative placement — often 0.25 to 0.3ml per side — is the standard approach, with a follow-up assessment at 2 weeks to evaluate whether additional product is warranted.
Under-eye filler lasts 9 to 18 months depending on the product used and individual metabolism. The periorbital area has active lymphatic drainage, which tends to break down filler faster than less-active areas like the cheekbone. Many clients find tear trough filler lasts 12 months on average before a top-up is needed.
It depends on the cause of the dark circles. Tear trough filler addresses structurally caused dark circles — the shadow cast by a hollow groove. If your dark circles are caused by volume loss and hollowing, filler can reduce them significantly. If your dark circles are caused by pigmentation (melanin deposits in the skin), thin skin showing the underlying vasculature, or chronic puffiness, filler does not address these causes. A consultation helps identify the cause — most clients have a combination of structural and pigmentary factors.
